Chronic, LLC., Certification Center provides assistance, education and the required physician certification to qualifying patients suffering from one or more of the listed conditions.  

 

Our doctors will review your medical records and consult with you on your current medical status.  If it is dertermined that medical marijuana may alleviate the symptoms or effects of your condition or treatment, you will recieve a signed Physician Certification Form as required to apply to the MMMP.

 

Our professional medical staff at Chronic, LLC., Certification Center can also assist you in renewing your Medical Marijuana Card, as required by the MMMA.  As of April 1st, 2013, all certifications are valid for 2 years.  Our committment to our patients includes strict adherence to and enforcement of all laws pertaining to HIPAA privacy protections.
